Self-Pay

Our self-pay package includes:

  • Surgeon fee, including initial consultation, personalized seminar and one year follow up
  • Hospital fee, including preoperative labs, EKG, Upper GI Series and postoperative Upper GI Series
  • Anesthesia fee
  • Psychological evaluation
  • Nutrition evaluation
  • BLIS insurance, minimal complication insurance
  • Preoperative liquid diet

For the above self-pay package, our prices are

  • Vertical Sleeve Gastrectomy$11,900*
  • Adjustable Gastric Band$13,900*
  • Roux-N-Y Gastric Bypass$22,000*
  • (*Prices slightly higher for patients with a BMI greater than 50)

For patients traveling long distances, we offer optional concierge program, which includes

  • Preoperative massage
  • Transportation to/from Airport & hospital
  • Hotel Stay (7 nights)
  • Hospital escort
  • Post-op protein supplements, 12 week supply
  • Arrange rental car, entertainment, dining

Concierge program: $1,500.00


Patient Information

Please bring the following paperwork to your appointment.
Your patient information form, as well as:
1. Medical Information: Please bring copies of any letters or medical records from other health care providers. Medical records for the past 5 years are necessary to file for insurance and to give the doctors a complete medical history.
2. Physician Recommendation: If filing for insurance, please provide a written recommendation for weight-loss surgery from your primary care physician.
3. Diet / Weight-Loss Info: Any documentation proving you participated in any diet / weight loss attempts. (This includes office notes, receipts or prescriptions.)
4. Insurance card
5. Driver's License
6. Physician Referral: If your insurance plan requires a referral for this consultation, please contact your primary care physician and have one faxed to our office fax at (704) 660-4967 before your appointment.


Are You Traveling?
We are convienently located thirty minutes north of Charlotte-Douglas Airport. We will work with you to schedule all of your evalutation appointments over a one-to-two-day period. We have contracted discount rates for our program participants with the following hotels:

Becoming a Patient

Step 1: Seminars

Attend Our Free Weight Loss Seminar

It is necessary to attend one of our free, monthly informational seminars before your first appointment with The Center for Surgical Weight Loss at Lake Norman. To reserve your spot in advance call (704) 660-4584. Space is limited.

Step 2: Insurance

Call Your Insurance Company
Check to make sure weight loss surgery is covered by your plan. Criteria for coverage will vary among insurance companies.

Step 3: See the Surgeon

See the Surgeon
After attending a seminar and deciding that this is something you would like to pursue, please call the office to schedule an appointment with the surgeon.

Step 4: Consultations

Consultations
To meet established guidelines for weight loss surgery, consultations with a registered dietician, a physical therapist and a behavioral health specialist are required.

Step 5: Pre-operative Visit

Pre-operative Appointments
You are moving forward with surgery and to prepare you for your new beginning, will need to attend the following appointment:

Support Groups

We offer support groups for post-surgery patients working toward their weight loss goals.

Support Group Schedule
Meets twice a month, the third Wednesday at 6 pm and fourth Thursday at 11:30 am at Lake Norman Regional Medical Center. Everyone is welcome. Call (704) 660-HLTH (4584).
